About Coordinated Care Agency
The Coordinated Care Agency is fully empowered to design and deliver innovative, welcoming, and comprehensive services and housing for people experiencing deep poverty and homelessness.
The purpose of our work is to ensure people have access to healthcare and supports they need to thrive. The team works to promote social changes to dismantle systemic racism, injustices and inequities that contribute to housing instability.
Guiding Principles of Coordinated Care
Everyone deserves housing
Everyone deserves housing.
It is everyone's job to ensure that people are moving towards obtaining and maintaining housing with the services they need - from the desk staff to the director.
We are a place of redemption
We are a place of redemption.
Everyone is better than the worst thing they have ever done - even if that is something they are doing to you right now.
Offer Benefit of the Doubt

We assume that our coworkers and our partners have good intentions especially when things seem to be going sideways in the relationship. We strive to understand things from their perspective.
We are Data Informed

Everything needs to be documented in the database so that we can communicate as a team, evaluate our results, and strive to improve.
We Thrive on Collaboration
We intentionally collaborate with other programs to share best practices and ensure coordinated, wrap around services are available.
We Strive for Equity and Racial Justice
We strive for equity and racial justice.
We acknowledge that racism and discrimination negatively impact our community and; we work to evaluate our work through an equity lens and seek opportunities for personal growth to reduce our biases.
