Foundational Community Supports

As part of the Washington State Medicaid Transformation Project, the Foundational Community Supports (FCS) includes Supportive Housing and Supportive Employment Services. This Medicaid behavioral health project offers a range of services, including:

  • Case management
  • Peer support
  • Skill building
  • Information and Referral
  • Hands-on assistance with job and housing search
  • Goal planning
  • Problem solving

Services are provided to individuals who are experiencing a number of complex health and risk factors. The goals are to connect people to the resources they need—like health care, income, social supports—in order to get and keep housing and employment.

The eligibility criteria for FCS is complex, and we are not always able to accommodate external referrals. Please call 206-798-2756 or send an email to FCSinfoKC@ccsww.org with inquiries about FCS eligibility or availability.

CCS is providing Foundational Community Supports in a wide variety of our programs within the agency. A Case Manager within the Rapid ReHousing program, Housing and Essential Needs, Regional Access Points or one of our shelters may ask for information to screen for FCS eligibility in order to provide this service for interested individuals.
